We provide a broad range of services in different locations of the state and tailor our services to meet the demands of the referral. The case manager or support coordinator assigned to the person will need to gather the following paperwork for new referrals to Tchanmeni Health Services:

Unannounced visits occur at least twice yearly, with announced visits each month. The personnel and providers of Tchanmeni Health Services work with the client's family, case manager/support coordinator, and other care providers as a team to ensure that the individual succeeds in the program.
Family members are urged to keep in touch with the individual receiving help
Visits are welcomed. When that is not feasible, images, letters, phone conversations, and even Skype are utilized to maintain ties from a distance.
Emergency assistance is accessible 24 hours a day
The Quality Assurance Team and Regional Coordinators are ready to assist a provider at any time of day or night to help resolve important occurrences. Naturally, all employees must contact local emergency services before taking further action in a medical emergency.
Paperwork
The paperwork for Tchanmeni Health Services is thorough, correct, and submitted on time. Quarterly reports are sent to Support Coordinators and replacement decision-makers. These reports provide a record of all attended medical visits and their results. Everyone is aware of what is going on in the service.
